Requirements:
- Accomplished Application Form for Marriage License (form);
- Original and three (3) copies of the contracting parties’ birth certificate issued by the Philippine Statistics Authority (PSA);
- Original and two (2) photocopies of PSA Certificate of No Marriage (CENOMAR) of each contracting party;
- Four (4) copies of the passports of the contracting parties;
- Two (2) recent passport-size pictures of the contracting parties;
- Notarized Affidavit of Parental Consent of the parents of party/ies aged 18-21;
- Notarized Affidavit of Parental Advice of the parents of party/ies aged 22-25; and
- Payment for Application for Marriage License ($45.00); and Service fee for Marriage Ceremony ($108.00)
The contracting parties must appear personally at the Consulate to submit the abovementioned documents and apply for a Marriage License. The Notice of Marriage shall be posted at the premises of the Philippine Consulate General for ten (10) consecutive working days, after which, the parties may get their Marriage License.
The contracting parties should return to the Consulate on the scheduled marriage ceremony.The Consulate sends the documents to the Department of Foreign Affairs in Manila every first week of each month for onward transmittal to PSA.
Correction of Entries of Civil Registry Documents (Report of Birth, Report of Marriage and Report of Death)

Filipinos abroad may file a petition before the Consulate to correct a clerical or typographical errors and change of first name or nickname, the day and month in the date of birth or sex of a person in the Civil Register Entries of the Consulate without need of a judicial order.
For those Civil Registry Entries not recorded by this Consulate, you can file your petition with the Civil Registry Section of the concerned Consulate/Embassy or Local Civil Registrar.

WHAT ARE THE CONDITIONS THAT THE PETITIONER THAT NEEDS TO COMPLY WITH?
For Change of First Name:
- The petitioner finds the first name or nickname to be ridiculous, tainted with dishonor or extremely difficult to write or pronounce;
- The new first name or nickname has been habitually and continuously used by the petitioner and he has been publicly known by that first name or nickname in the community; or
- The change will avoid confusion.

For Correction of Clerical/Typographical Error:
- The error must be visible to the eyes or obvious to the understanding, and can be corrected or changed only by reference to other existing record or records.

WHO MAY FILE THE PETITION?
Whether it is for correction of clerical or typographical error, or for change of first name, the petition may be filed by a person above 18 years of age and must have a direct and personal interest in the correction of the error or in the change of first name in the civil register.
Only the following persons are considered to have a direct and personal interest in the correction of clerical error or change of first name:
- Owner of the record that contains the error to be corrected or first name to be changed.
- Owner’s spouse, children, parents, brothers, sisters, grandparents, guardian, or any other person duly authorized by law or by the owner of the document sought to be corrected.
WHAT SHOULD BE THE FORM AND CONTENT OF THE PETITION?
The petition must be submitted in person, whether it is for correction of clerical error or for a change of first name, should be accomplished properly and in the prescribed form.
The law requires that the petition should be in the form of an affidavit, hence, it should be subscribed and sworn to before a person authorized to administer oath.
Note that the Petition and its supporting documents must be filed in four (4) copies.
WHAT SUPPORTING DOCUMENTS ARE REQUIRED FOR CORRECTING A CLERICAL OR TYPOGRAPHICAL ERROR IN A CIVIL REGISTRY DOCUMENT?
The petition shall not be processed unless the petitioner submits the required documents. The supporting documents should be authentic and genuine; otherwise, the petition shall be denied or disapproved. The following supporting documents are admissible as basic requirements:
- Certified machine copy of the certificate containing the alleged erroneous entry or entries.
- Not less than 2 public or private documents upon which the correction shall be based. Examples of these documents are the following: baptismal certificate, voter’s affidavit, employment record, GSIS/SSS record, medical record, school record, business record, driver’s license, insurance, land titles, certificate of land transfer, bank passbook, NBI/police clearance, civil registry records of ascendants, and others.
- Certified machine copy of the Official Receipt of the filing fee.
- For correction of sex, the additional documents are:
- medical certification issued by an accredited government physician that the petitioner has not undergone sex change or sex transplant.
- Affidavit of publication from the publisher and a copy of the newspaper clipping.
- Clearance from authorities such as clearance from employer, if employed; the National Bureau of Investigation; the Philippine National Police and other law enforcement agencies where person whose day and/or month in the date of birth is erroneous resides.
- Affidavit of publication from the publisher and a copy of the newspaper clipping.
- For correction of day and/or month in the date of birth, the additional documents are:
WHAT ARE THE SUPPORTING DOCUMENTS FOR CHANGE OF FIRST NAME?
As in the case of correction of clerical error, no petition for change of first name shall be accepted unless the petitioner submits the required supporting papers, as follows:
- All the documents required of the petitioner for the correction of clerical error shall also be required of the petitioner for change of first name.
- Clearance from authorities such as clearance from employer, if employed; the National Bureau of Investigation; the Philippine National Police and other law enforcement agencies where person whose name is being changed resides.
- The Consulate’s Civil Registry Section may require additional supporting papers.
- An affidavit of publication from the publisher and copy of the newspaper clippings should be attached.
HOW MUCH IS THE FILING FEE FOR THE PETITION?
The fees are the same for all Philippine Consulates. The fees are the following:
- AU$90.00 for the correction of clerical or typographical error.
- AU$170.00 for the change of first name.

FORM
Report of Death shall be submitted by mail to the Consulate
When reporting/registering the death of a Filipino citizen, the following documents should be submitted by mail:
- Duly accomplished Report of Death (4 original);
- Original and 4 photocopies of the following:
- Death Certificate issued by the Australian Registrar of Births, Deaths and Marriages;
- Photocopy of Data page of the Philippine passport of the deceased;
- Mortuary Certificate* issued by the Philippine Consulate General (for shipment of human remains or urns for repatriation to the Philippines)
- Processing fee of AU$45.00 in postal money order made payable to the Philippine Consulate General. Do not send cash in the mail.
Requirements for Consular Mortuary Certificate:
- Death Certificate issued by the Australian Registrar of Births, Deaths and Marriages;
- Embalming Certificate, if the remains are embalmed;
- Cremation Certificate, if the remains are cremated;
- Travel itineraries for the remains and the one who will accompany the remains;
- Original and photocopy of the Philippine passport of the deceased;
- Medical Certificate of Death;
- Burial or transit permit for shipment of human remains;
- Mortuary Certificate stating that the casket contains only the body of the deceased or certification from crematorium stating that the urn contains only the ashes of the deceased.
- Processing Fee: AU$45.00 in cash or postal money order made payable to the Philippine Consulate General. Do not send cash in the mail.
- Self-addressed Express Post envelope with contact details for the return of the ROD.
RELEASING
- The personal copy of the ROD will be sent by mail the next working day from receipt of complete documents
- The Consulate assumes no responsibility for any delay, loss or damage in the mail or while the documents are in custody in courier service. The applicant should note the tracking numbers of all envelopes used and submitted.
The Philippine Consulate General will transmit the ROD to the Philippine Statistics Authority (PSA). There is a waiting period of at least 4 to 6 months for the availability of PSA copy in security paper. Please get it from PSA by providing the transmittal details (Despatch number, Reference Number and Date of transmittal).

REPORT OF BIRTH
The birth of a child born in Australia of Filipino parents should be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Consulate General.

When reporting/registering the birth of a Filipino citizen, the following documents should be submitted:
Requirements:
- Duly accomplished Report of Birth Form (4 originals). The form must be typewritten. Kindly ensure that all information is accurate to prevent the delay in the transmittal to (PSA). If submitting by mail, the form shall be notarized.
- Original and 4 photocopies of the following:
*if by mail, documents must be certified true copy (CTC). Do not send original documents by mail.
- Australian Birth Certificate issued by the Registrar of Births, Deaths and Marriages
- Photocopy of Philippine Passports of Parents
- Parent/s proof of status, i.e., Visa issued by Australia, Dual Citizenship documents
- Philippine Statistics Authority (PSA) issued Marriage Certificate of Parents (if married) or Report of Marriage of Parents (if married overseas)
- If Parents are not married and the child will use the surname of the Father, the following are required:
- Father shall execute an Affidavit of Admission of Paternity (AAP) (form)
- Mother/Guardian shall execute an Affidavit to Use the Surname of the Father (AUSF)(form)
- If child is 0-6 years old; AUSF to be executed by mother
- If child is 7-17 years old; AUSF to be signed by the child with Sworn Attestation of Mother/Guardian
- If child is 18 years old above; AUSF to be executed by child with no required Sworn Attestation
- If Parents were married only after the child’s birth, submit an Affidavit of Legitimation (form) with PSA issued Birth Certificate of Mother.
- Processing Fee: AUD$45.00 in postal money order made payable to the Philippine Consulate General. Do not send cash in the mail.
*Please note that each affidavit has an additional fee of AUD$45.00

Additional Requirement:
Notarized Affidavit of Delayed Registration of Birth (for Delayed Reporting 1 year after the date of birth).
The Consular Officer reserves the right to require additional proof of documents from an applicant to prove his/her citizenship, status or identity pursuant to the Philippine Passport law (RA 8239) and the Foreign Service Act (RA 7157).
